1) Enter the First and/or Last Name of a Provider.
2) To select a provider from the suggestion list, use your down/up arrows and press Enter OR use your mouse to click on the provider.
3) If you don't see a provider in the suggestion box enter a few letters (at least one letter is required) and your search results will return any providers where the first or last name begins with your search criteria.
